Acceptance of Terms
By accessing or using the website located at mydentistchandler.com (the "Site") or making a payment through our online payment portal, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use the Site or submit any payment.
These Terms apply to all visitors, patients, and others who access or use the Site.
Services and Appointments
Anantuni Family Dental & Aesthetics provides dental care services at 5590 W Chandler Blvd., Suite #1, Chandler, AZ 85226. The Site allows you to request appointments, learn about services, and submit payments for dental services rendered.
Appointment requests submitted through the Site or third-party booking tools are not confirmed until you receive direct confirmation from our office. We reserve the right to reschedule or cancel appointments as needed.
Online Payments
Payment Processing
Payments submitted through our online payment portal are processed by a third-party payment processor. By submitting payment information, you authorize Anantuni Family Dental & Aesthetics to charge the provided payment method for the amount indicated.
Accuracy of Payment Information
You agree to provide accurate and complete payment information. You represent that you are authorized to use the payment method provided. We are not responsible for declined transactions or fees charged by your financial institution.
Refunds
Refund requests are handled on a case-by-case basis in accordance with our office policies. Please contact our office directly at (480) 660-4598 or office@anantunidental.com to request a refund. Overpayments will be applied as a credit to your account or refunded upon request.
PCI DSS Compliance
Anantuni Family Dental & Aesthetics complies with the Payment Card Industry Data Security Standard (PCI DSS) as a merchant that fully outsources card processing to a PCI DSS-validated third-party payment processor. We do not collect, transmit, or store cardholder data on our own systems or website. All payment card information is entered directly into our payment processor's secure, PCI-compliant environment.
Third-Party Payment Processor
Online payments are processed by LQpay (Liquid Payments, Inc.), a healthcare payments provider that maintains PCI DSS compliance for the protection of cardholder data. When you click "Pay Now" on our website, you are redirected to LQpay's secure payment portal at anantunifamilydental.secure.lq-pay.net, where your card information is encrypted in transit using TLS and processed in accordance with PCI DSS requirements. LQpay's privacy and security practices are governed by their own policies, which we encourage you to review.
Card Data Handling
We do not see, store, or have access to your full credit card number, expiration date, or CVV. Our office may retain a tokenized reference (a non-sensitive identifier issued by LQpay) and the last four digits of a card on file solely to process patient-authorized charges. You may request that any saved payment method be removed at any time by contacting our office at (480) 660-4598.
Reporting a Security Concern
If you suspect unauthorized use of your card or a security issue with our payment process, contact your card issuer immediately and notify us at office@anantunidental.com so we can coordinate with our payment processor.
SMS/Text Messaging Terms
When you opt-in to the service, we may send you a message to confirm your signup. Message and data rates may apply. Message frequency varies. Text "HELP" for help. Text "STOP" to cancel.
You can cancel this service at any time. Just text "STOP" to our phone number. After you send the message "STOP" to us, we will reply to confirm that you have been unsubscribed. After this, you will no longer receive messages from us.
If at any time you forget what keywords are supported, just text "HELP" to our phone number. After you send the message "HELP" to us, we will respond with instructions on how to use our service and how to unsubscribe.

Carriers are not liable for delayed or undelivered messages.
